Our hospital to home program provides a 360-view of a patient’s health following an ED event. We collaborate with our members’ external providers to fill in the gaps—allowing members to adhere to their health plans.

Lean on us for house calls and get relief for common conditions, such as:

  • Severe or ongoing cough and cold
  • Nausea, abdominal pain, or diarrhea
  • Ongoing muscle or joint pain
  • Cuts, scrapes, or wounds
  • A burning or constant urge to pee, or if you have dark or cloudy urine
  • A fever over 100° or that hasn’t gotten better with medicine

Get urgent care over the phone or at home

Cityblock members can call us first, anytime, for consultations. For situations that require it, our care team can be at your doorstep. Some of the services we are fully equipped to provide are:

  • Checking for vital signs such as your temperature, blood pressure, and blood oxygen levels
  • Treatment for pain and discomfort
  • Administering and reading EKG results
  • Testing for: A1C, the flu, COVID-19, strep, RSV, pregnancy
  • Oxygen administration
  • IV hydration, antibiotics, nausea medications, and non-narcotic pain medications
  • Prescribe medications for treatment
  • Wound care, and more
