We look to understand your strengths and to identify the challenges you face, then we build a team of expert staff to provide you with better care that’s tailored specifically for you. We know that good care isn’t just about what happens at the doctor’s office, it’s about making sure you have access to transportation, housing, nutritious food, and all the other building blocks that help you to stay healthy.

As a member, you’ll have your own team that’s tailored to your needs. You can connect anytime in the ways most convenient for you, from meeting at our Neighborhood Health Hubs to communicating via phone or computer to scheduling visits at your home or another location of your choosing.

Each team has its own physician to help you stay as healthy as possible, and to support you with any medical issues you're concerned about. Because we build our care around you, appointments are never rushed. And if you need a specialist, our team can help make sure you're in the best hands, and will work with you and that doctor on your follow-up plans.

Behavioral Health

We have trained psychiatrists, substance use specialists, and therapists on staff to listen and help with your health challenges, whether it’s meeting weight-loss goals, kicking a habit or addiction, dealing with anxiety or depression, or coping with life events or family problems.

Day-to-Day Support

Your care team will be led by a dedicated Community Health Partner matched to your specific needs. Your Community Health Partner is invested in you and your story, and will be ready to assist with anything you need to meet your goals and thrive—from medication to housing to family counseling.

A Neighborhood Health Hub built to support your experience:

Our Hubs aren’t just our headquarters. They’re places where you and your community can get stuff done. While they’re all easily accessible by foot or public transportation, we also help arrange transportation to the Hub and back if you ever need a lift.

You can meet with your care team in our consultation room and fill prescriptions right away. You’ll also find local community-based organizations on-site to help you manage specific needs, like benefits assistance.

Your family

Our goal is to offer a warm and supportive environment for parents and children alike. We recognize the importance of accessible childcare services, help with short-term housing needs, and education on keeping the whole family healthy. In addition, you’ll have access to support groups, family counseling, and family meetings with care teams.

From fitness classes to resume workshops to music concerts, we book a range of health, educational, and social events. At each Hub you can also connect with a variety of services tailored to that community’s needs.

Using Commons, your Community Health Partner will walk through straightforward questions to develop a complete view of the challenges you face—not just in your health, but also across all dimensions of life. This will help us figure out how we can best support you.

Through the platform, you and your care team will develop a Member Action Plan to set clear goals together. We get your opinions from the start, and over time we make changes quickly based on your feedback. This is your MAP for your health.

Mobile communication

We want to make it as easy as possible for you to stay in touch. You can chat with us securely by text message, email, phone, or video 24/7 — truly, we’ll be there in the middle of the night. All are user-friendly and private.
