We’ll spend time with you to hear your story and to understand your priorities for health and daily life. Together, we’ll then build a plan to get you the care and services that will work for you.
Cityblock comes at no additional cost to you if you are eligible through your health plan. Copays, coinsurance, and deductibles apply to services covered by your health plan.
We’re currently seeing eligible EmblemHealth members in New York and ConnectiCare members in Connecticut.

Every Cityblock member is supported by a care team that works closely together to ensure all of your goals are heard and addressed. And they will meet you wherever and whenever is convenient for you—at home, at a convenient place where you’re comfortable, like a community center or a church, or by phone or video visit.
Our care teams include doctors, nurse care managers, and others to support you with any medical conditions or symptoms you’re concerned about—we’ll always make time for you. If a specialist would be helpful, our team will connect you with the right one, so you get care that works for you.
Being healthy isn’t just about physical conditions—how you’re doing mentally and emotionally is just as important. Our psychiatrists and therapists are there to listen and help, be it treating anxiety, depression, or substance use issues, managing a chronic condition like diabetes, or other stressors you are experiencing.
You will have your own Community Health Partner to support you in addressing problems big and small—helping to coordinate transportation, fill out applications and help you take care of your daily needs. They are ready to support you with whatever you need to meet your goals for your health and well-being.
Being a Cityblock member means you have a care team that has your back at all times. We listen to understand what your daily life looks like, and what your goals and priorities are. You have the support of your Community Health Partner and your care team, and can easily access an array of community services and resources with Cityblock’s help. As part of the Cityblock community, great care is easy, supportive, and useful, every day.
We’ll confirm eligibility with your health insurance plan and that we’re available in your neighborhood.
A care team member will be in touch to set up a time for an initial sit-down so we can get to know each other.
We’ll then work together to put the right people on your care team and introduce you to them.
Your care team will spend time with you and create a personalized Member Action Plan.
Over time, you and your care team will work towards your goals for your health and daily life.
There’s no endpoint with Cityblock care. We’ll continue to learn and address your goals and priorities over time.
Our Neighborhood Hubs are so much more than traditional clinics where you go for doctor’s appointments. We design them specifically to meet your needs. They are places where you can get support for what’s going on in your life, but also a community gathering space where you are welcome to drop in for as long as you would like.
At your Neighborhood Hub, you can meet with your care team, connect with local community services, or just drop by and say hello.
Our goal is to provide a supportive and welcoming environment for you and your family. At the Hub, you’ll have access to support groups, family counseling, and family meetings with care teams.
From job training to fitness classes, we facilitate a range of health, educational, and social events. Through each Hub, you can also meet with local organizations that meet your interests and what’s going on in your daily life.
