You’re not alone

Lots of people struggle with their mental health at some point. Sometimes these struggles last just briefly, and sometimes they’re part of your daily life. Think about the statements below. Do any of them describe you?

  • “I can’t sleep.”
  • “I feel totally overwhelmed.”
  • “I’ve lost interest in things I used to enjoy.”
  • “I use alcohol, tobacco, or another substance more than I want to.”
  • “I’m feeling really down. I wish I had someone to talk to.”
  • “I find myself crying a lot.”

If the answer is yes for any of these, it’s nothing to be ashamed of. There are health services that can help. And Cityblock is here to help you find them.

A team you can count on

All members of your Cityblock care team talk to each other and know your health needs. This means your care across providers will be consistent, and you won’t have to tell your story over and over. Your Cityblock team may also bring on licensed behavioral health specialists if you need it.

A bridge to long-term stability

It can be difficult to find the right mental health provider or community program. Cityblock focuses on filling in that gap with meaningful treatment. We’ll work with you as long as it takes to ensure you’re safely transitioned to the right kind of care for you

More than just referrals

When you need a referral, we don’t just send in the paperwork. We help you decide on the right provider and make sure they’re covered by your insurance. We have relationships with great providers, so when we refer you, it’s to someone we believe will give you excellent care.

Extra support and guidance

Your Cityblock care team, and especially your Community Health Partner, are here to support your mental health outside of the doctor’s office. We can help with scheduling appointments, setting up video calls, or just provide someone to talk to.

Member story: George

George had struggled with his mental health for a long time. But he didn't feel comfortable asking for help.

“I’ve got a lot of things covered up, a lot of things that’s hid,” he explained.

George’s Cityblock Community Health partner reached out to him consistently, without judging or rushing him. George felt that he was being listened to and respected. He opened up about his life and sought treatment.
