We can help with housing, employment, legal aid, food, childcare and other services you may need.
Mental health care
We provide a wide range of behavioral health services, including counseling, psychiatry, and integrated healthcare for substance use disorder and serious mental illness.
Care coordination
We work with our members’ doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.
Complex care management
Our care teams are experienced at working with members who have chronic conditions and disabilities. We help members reach the health goals that matter to them.
Urgent care that comes to you
We can be at your doorstep for non-life-threatening situations* such as the flu, nausea, abdominal pain, chest pain, general pain, cuts, or burns.

Community Health Partners (CHPs)
Although you may see a team of providers with us, one thing will always remain the same—they’ll be available when you need them, and will never put a time limit on your care. They will also work with your current doctors, or if you have one that you trust, so there won’t be any gaps in your care.
