Staying Healthy After You Leave the Hospital
Leaving the hospital is a big step toward feeling better! To keep you feeling well and out of the hospital, here are the 5 most important things to do once you get home.
1. Know Your Medications
- Take your medicine exactly as your doctor told you.
- Keep a list of all your medicines and when to take them.
- If you’re not sure what a pill is for, ask your nurse, doctor, pharmacist or Cityblock care team before you take it.
- Don’t skip doses or stop a medicine without asking first.
- Make sure you get all your new medicines. Ask your family, friends or Cityblock care team to help if you need it.
2. Follow Your Care Plan
- Find your hospital papers (your discharge instructions) and keep them where you can easily see them.
- Write down all your follow-up appointments. Call your Cityblock team at 833-904-2273 if you need to connect to resources for a ride, need help scheduling, or if you can't make it.
- Always bring your list of medicines and your hospital papers to your next doctor or therapist visit.
3. Watch for Warning Signs
- Pay attention to your body and how you feel. If you start having the same symptoms that sent you to the hospital, don't wait.
- Avoid triggering places, people or things.
- If you feel hopeless, worthless, or just really down, please reach out.
- Know who to call early: It's always better to ask early than to wait until it gets worse.
- Call your Cityblock team at 833-904-2273.
- For a crisis, you can call or text 988.
- In an emergency, call 911.
4. Take Care of Yourself
- Eat healthy foods and drink plenty of water.
- Rest when you need to, but try to move a little each day, too (if it's safe).
- Try to avoid smoking, drugs, or alcohol, as they can make it harder to heal.
5. Ask for Help
You don’t have to do this alone!
