So much of what happens in daily life affects health, from caring for others to where you live and having access to transportation and nutritious food. We get that. With Cityblock, every member is supported by a care team, led by a Community Health Partner, who ensure members have the care and support they need for their health and day-to-day priorities. Our care teams meet members whenever and wherever is most convenient—at home, in the community, even by phone or video visit.
How much will this cost? Cityblock's services are available to members at no additional cost and with no changes to their current health plan benefits.*
How do I know if I can join Cityblock? You’ll receive a letter from ConnectiCare and Cityblock in the mail, followed by a phone call from a Cityblock team member. If you have questions about membership, you can call ConnectiCare at 1-800-390-3522 (TTY: 1-800-842-9710) Monday through Friday, 8 a.m. to 4 p.m.
Will I need to see a new doctor? If you already have a primary care provider, Cityblock’s care teams will work closely with them on your care needs.
*There is no change to your health plan benefits when you join Cityblock Health. Copays, coinsurance, and deductibles apply to services covered by your health plan. Cityblock-branded medical practices are independently owned and operated by licensed physicians. Other providers are available in the ConnectiCare network.
ConnectiCare will contact you first by mail to let you know that you may join Cityblock. Then we'll reach out to welcome you and schedule a time to meet. We would also love to hear from you! Call us anytime at (203) 518-4566 or for TTY, use 711.
We will then match you with a designated Community Health Partner (CHP). Starting with your first meeting, you will assess your priorities, create an action plan, and work together towards better health and well-being.
Your CHP will bring in a full care team—which may include a doctor, nurse care manager, behavioral health specialist, and others—to help you achieve your goals together over time.
