Our members have access to a dedicated care team of nurses, doctors, and other providers in Massachusetts for primary care, mental health and more. These services can be provided in the comfort of your home, or preferred location. Primary care
We coordinate with members’ existing providers, or assign physicians based on your specific needs.
24/7 care line
Because health doesn’t have business hours, members have a direct line to a provider when they need medical advice, 24/7.
Behavioral health
We provide a wide range of behavioral health services for members, including counseling, psychiatry, and integrated healthcare for substance use disorder and serious mental illness.
Social services
We can help with housing, employment, legal aid, food, child care and other services members may need.
Care coordination
We work with our members’ doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.
Complex care management
Our care teams are experienced at working with members who have chronic conditions and disabilities. We help members reach the health goals that matter to them.
Lab draws
Our care team can perform members’ blood work at home.
