Our members have access to a dedicated care team in Florida. Cityblock services can be provided in the comfort of the home, virtually, or at a preferred location. Cityblock members can book an appointment with their care team by calling 833-904-2273 (TTY 711).

Mental health
We provide a wide range of behavioral health services for members, including counseling, psychiatry, and integrated healthcare for substance use disorder and serious mental illness
Social care services
We can help members access support for needs such as housing, employment, legal aid, food, child care and other services members may need.
24/7 Care line
Because health doesn’t have business hours, members have a direct line to a provider when they need medical advice, 24/7.
Care coordination
We work with our members’ doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.
Complex care management
Our care teams are experienced at working with members who have chronic conditions and disabilities. We help members reach the health goals that matter to them.
Lab draws
Our care team can perform members' blood work at home.

Partnering with insurance companies reduces long-term healthcare costs for our members such as hospital stays and emergency room visits.
